First Time Visits for New Patients

Make sure your first visit to Urology Associates of Grand Rapids, PC, is as smooth as possible, with the tips provided on this page. Before your visit, please make sure you download and complete the patient forms, to the right, if you were unable to mail them to the office prior to your first appointment.


About your visit

Our staff will make every effort to schedule an appointment with the physician of your choice. If that particular isn't available, the staff will suggest one of our other qualified providers for you to consider.

New patients

You can schedule an appointment at any of our locations, Grand Rapids, Grant, or Greenville offices. Our staff will ask you questions about your medical condition to ensure there is enough time for your visit.

Anything you tell us will be kept confidential for your protection per the HIPAA rules and regulations.

What to bring to your first visit

  • A valid driver's license or government issued picture ID
  • Insurance cards
  • The completed forms you downloaded below or that were mailed to your home

Please be prepared to pay your deductible or co-pay

Insurance information

Many insurance plans require a referral to our office. We recommend that you alway keep your primary care physician informed of any ongoing urgent and non-urgent medical issues.

If a referral is required and not in place for your first appointment, you will be responsible for payment at that time. You should also check with your insurance provider to make sure your urologist is on the list of in-network participating providers. Be familiar with the rules, limitations, co-pays, deductibles and/or patient responsibility required by your insurance company. You will be responsible for paying any financial balance your insurance does not cover.
